Accidents
Theme parks and roller coasters are statistically very, very, very safe. Out of the millions of people who ride the thousands of rides every year, there are only a small handful of incidents. You are statistically more likely to be involved in an accident driving to an amusement park than having one at a park. The rarity of accidents is one reason why the media report them, similar to plane crashes.
Every important element of a ride is inspected on a regular basis and every ride is tested every day without passengers before it opens. A ride might not open, or a safety system might activate, if there is an imperfection. For theme parks to operate, they must obtain insurance and one stipulation is their rides are frequently inspected and signed off by a strict Health & Safety governing body. Every theme park does what they can to prevent accidents because the negative press would be bad for business.

A worker was crushed by the large counter weight attached to the ride when he entered a danger zone at the bottom of the ride in an attempt to retrieve a tape measure. Source
Ninja, Six Flags Magic Mountain, California, USA
A 20-year-old man was hit by the Arrow suspended coaster when he entered a restricted area, climbing two fences to retrieve a lost hat. He was treated at the scene by park first-aid, then air-lifted to hospital in comatose condition and pronounced dead the next day. Source
Rainbow, Liseberg, Gothenburg, Sweden
The HUSS magic carpet ride collapsed, causing some riders to be thrown from the platform. There were no serious injuries, although three riders were hospitalized. Source

A male worker was struck by one of the E&F Miler Industries coaster cars while attempting to retrieve a lost hat, and died later in a local hospital. Source

Four people were injured when the Sartori Energizer inverted coaster‘s car derailed and fell to the ground. Source

Two riders were thrown from the ride when the seating collapsed. Both suffered back injuries and bruising.

A 17-year-old was decapitated when he entered a restricted area to retrieve his baseball cap.
Wildcat, Cedar Point, Ohio, USA
Two cars collided resulting in nine riders being injured.
Motion Master, Flamingoland, UK
A 2-year-old boy fell through a hole in the ride’s floor onto concrete 8 feet below. He suffered only minor injuries.
